In pursuance of the Hon'ble Court Judgment dated 30.06.2021 in W. P (C) No-539/2021 and W.P (C) No. 554/2021 and NSDMA Letter No. NSDMA-SDRF-CExG/383/2021/5669, Kohima, dated the 18th October 2021, Deputy Commissioner, Kohima, Gregory Thejawelie, NCS has informed the concerned next of kin of the deceased due to COVID-19 that Ex-gratia forms (Annexure -A) and checklist of required documents can be availed from DDMA Cell  Office of the Deputy Commissioner, Kohima during office hour.
