In pursuance of the letter from the Ministry of Social Justice & Empowerment, Department of Empowerment of Persons with Disabilities (Divyangjan), the Deputy Commissioner Mokokchung has informed that an Assessment Camp for distribution of aid and assistive devices by ALIMCO under Rashtriya Vayoshee Yojan (For Senior Citizens) and Assistance to Disabled Persons of Purchase (ADIP) Schemes has been scheduled to be held on the following dates and Venues as Mentioned.

5th July 2022 – IMDH, Mokokchung at 10:00 am

6th July 2022- CHC, Mangkolemba at 10:00 am

7th July 2022- CHC, Changtongya at 10:00 am

8th July 2022- CHC, Tuli at 10:00 am

Further, the following documents are to be produced (Mandatory) by the beneficiaries on the day of the Assessment Camp. For Rashtriya Vayoshree Yojna (Senior Citizens)

1. Passport size Photo
2. Copy of Age & Address Proof (AADHAR Card\Voter ID) or any valid Government ID Card.
3. An income certificate (Equal to or below Rs. 15000/ Month) issued by an appropriate authority (any Gazette Officer)
4. Photo Copy of Proof of receiving Old Age Pension under IGNOAPS or any other state Government Pension for BPL Categories people.

For Assistance to Disabled Pensions for purchase (ADIP) schemes

1. Passport Size photo Showing disability.
2. Photocopy of Address proof (AADHAR Card\Voter ID) or any valid Government ID Card
3. An income certificate (Equal to or below Rs. 15000/ Month) issued by an appropriate authority (any Gazette Officer)
4. Disability certificate having disability 40% or above.
