The DC & DEO Tuensang, Nokchasashi has informed all the concerned engaged in election-related works such as ROs, AROs, Sector Officers, employees posted in the office of the Deputy Commission & District Election Officer Tuensang, Control Room, NIC, all EEM teams, Micro Observer, Liaison Officers, DLMTs and any other details in election-related works by the DEO Tuensang to collect Form 12 from the Postal Ballot Cell, Tuensang. It was also informed that the form should be submitted on or before 8th February 2023. A copy of EPIC or any other relevant documents should be produced while applying for the postal ballot in Form 12 approved by the commission, which are:

  1. Aadhaar card  
  2. MNREGA Card
  3. Passbook issued by banks and the Post Office.
  4. Health Insurance Smart Card issued under the schemes of the Ministry of Labour.
  5. Driving license
  6. Pan Card.
  7. Smart card issued by RGI NPR
  8. Indian passport
  9. Pension Paper with Photograph.
  10. Service identity card with photograph issued by central/state /govt/PSUs/Public Limited companies 
  11. Official Identity card issued to MPs/MLAs/ML Card
  12. Unique disability ID (UDID)
