Deputy Commissioner & District Election Officer, Wokha Ajit Kumar Ranjan (IAS) informed all persons engaged in election-related works such as Returning Officers, Assistant Returning Officers, Sector Officers/Magistrates, employees posted in the Office of the DEO/RO, control Room, NIC all EEM teams, micro Observers, Liaison officers, DLMTs, ALMTs, BLOs appointed outside their polling station and any other detailed in election works by the District Election Officer Wokha to collect Form12 to facilitate the issuance of Postal ballot.

The form may be collected from the Postal Ballot Cell at the Office of the DC & DEO Wokha. Filled in Form 12 should be submitted on or before February 2023 with a photocopy of the EPIC card or in case of non-availability of the EPIC card, any one of the 12 identification documents as notified by CEO Nagaland may be submitted (photocopy)-

Aadhaar Card, MNREGA job card, Passbook with photograph issued by Bank/Post Office, Health Insurance smart card issued under the scheme of the Ministry of Labour, Driving License, PAN card, Smart card issued by RGI under NPR, Indian Passport, Pension documents with photograph, Service Identity cards with photograph issued to employees by Central/ State Govt./PSUs/ Public Limited Companies, Official Identity cards issued to MPs/MLAs/MLCs, Unique Disability ID (UDID) card, M/O Social & Empowerment Government of India.
