The Directorate of Treasuries & Accounts have notified that the Life Certificate for Pensioners issued by Gazetted Officers and submitted to Treasuries & Banks is hereby amended and will now be collected only as per the new format.

Treasuries & Banks will henceforth accept information of the pensioners only as per the enclosed format. Additional information, if required by the banks, may be collected separately.

Amendment of the format is not permitted and submission of Life Certificate in any other format will henceforth be considered null & void. The new format will be effective from the date of issue of this notification.
