Deputy Commissioner Mokokchung, Sashank Pratap Singh, IAS, has informed that a Beneficiary Enrollment Drive for Chief Minister’s Health Insurance Scheme (CMHIS) will be conducted starting from 18th October to 31st October 2022, at the Registration Centre, CMHIS located at the Deputy Commissioner’s Office, Mokokchung.

The mandatory KYC documents required to enroll for General Category (including employees without PIMS number, contractual, adhoc, fixed pay employees of the state and contractual employees under various CSS, with IIC and/or PRC) under the scheme shall be as follows:

1.            Indigenous Inhabitant Certificate (IIC) or Permanent Residential Certificate (PRC) issued by the District Administration, Mokokchung after 2016.

2.            Valid Mobile Number.

3.            Ration Card No (for Ration Card Holders).
